Visionmaker Jamaica Bay Surveys and Consent Form

Sanderson E. Visionmaker Jamaica Bay Surveys and Consent Form

Before and after each workshop, participants were encouraged to fill out pre- and post-surveys about their experience. Surveys were conducted on-line and could be filled out at the workshop site since participants had computers and Internet access in order to use Visionmaker. The surveys enabled us to assign a unique identifier to each vision and connect it to an individual, though data analysis was conducted without any personally identifying information. Prior to the workshop, we received approval from the Cornell University Institutional Review Board (IRB) because our research included human subjects; we abided by all the rules of Cornell University with regards to this research. Participants were asked to sign a consent form informing them about the nature of the research and how their data would be used. Visionmaker NYC Jamaica Bay documents to be completed prior to and after workshop: 1. pre-survey form 2. post-survey form 3. consent form
